Yes, if you click on kid there is a blue button that says makeover and then you can do it!

You can give your child a makeover, press on your kid and on the bottom left there is a blue button that says makeover!

You can change it by making him a playable Sim

Yes you can, but your child should be a playable sim. (Slotted and not npc) Taking into consider that the child becomes an adult. Any age except adults can be considered as NPC's as we can't really edit them in cas or play them.
