Missing medical career option

I'm level 23 and idk how i lose my medical career. The suitcase is just gone, i've never started it but it seems i never will be able to.

Could it be that your sim is retired? I read that if you don’t retire when you have the option it will automatically retire after 2 days

have you tried to just go to the hospital on see if you can enter or go to you sims icon and look at careers and see if you can join there?
